Thank you for your time at CrossFit Plainfield, it was a pleasure having you here. We are sorry to see you go!  

If you decide to reactivate your membership in the future (and we hope you do) membership rates in effect at the time of reactivation will be applicable, as membership rates are subject to change.

If you are sure you’d like to cancel, this form MUST be completed prior to membership cancellation. This serves as your 2 week written cancellation notice as required by your Membership Agreement.  Your membership will be cancelled 2 weeks from submission of the form below.  Please note: If you have a scheduled payment within this period, payment will be processed as scheduled.  All payments are non-refundable.

If you would like to meet with a Coach to schedule a Quarterly Check-in to discuss other options because you know how important your health and fitness are, contact the gym now.
